The Ligue 1 side lost 2-0 to Napoli, 4-0 on aggregate, with Napoli’s first goal coming three minutes into the second half while Balotelli was arguing with match officials on the touchline.
Mario Balotelli was roundly criticised by his coach as Nice failed to qualify for the Champions League group stage.
The row appeared to centre around a white bandage on his wrist which French media said was covering up a bracelet, which is banned under the laws of the game.
While Balotelli was arguing, forward Jose Callejon was left unmarked at the far post and scored easily.
Nice coach Lucien Favre reacted furiously to the goal while Balotelli continued his discussions with the officials.
The former Manchester City and Liverpool striker was slugglish throughout the game and was eventually substituted to jeers from the Nice fans.
Lorenzo Insigne wrapped things up with Napoli’s second goal a minute before the end.
Afterwards, Favre said: “I don’t want to comment on Balotelli. It wasn’t very satisfactory, it was not good enough.”
Favre also complained that Balotelli was too slow in leaving the field when he was substituted.
Nice will now enter the Europa League group stage while Napoli are safely into the Champions League.
